Features exploration and service provision strategies for long-term care beneficiaries with dementia

Abstract
As the continuing growth of the population with dementia, the Korean government has focused a national responsibility for dementia care. Especially, it has been empathized that personalized long-term care (LTC) services based on their health and functional status are provided to person with dementia. This study aimed to derive features for beneficiaries with dementia and to examine LTC services utilization according to the features. Finally, we suggested strategies to provide personalized LTC services for beneficiaries with dementia. Study subjects were 275,230 and we used the LTC insurance database including applicants¡¯ assessment data and beneficiaries¡¯ LTC service utilization.
The cluster analysis was performed to derive features for beneficiaries with dementia, and descriptive statistics were conducted to explore the utilization of LTC services during 6 months. A total of 7 features were derived: feature1-3 of mild cognitive function (CF) and feature4-7 of above moderate CF. In similar CF states, activities of daily living, intrumental activities daily living, and behavioral and psychological symptoms (BPS) were heterogeneous, and the states of CF and BPS were different within each LTC benefit level. There were no significant differences between the 7 features in the types of LTC service utilization. Effective strategies of personalized LTC services for beneficiaries with dementia are to prepare an information collection system including the various health and functional status. In addition, various LTC services for beneficiaries with dementia should be developed.
